🥁 We are excited to share that the new Child Health Referral Form (an updated version of the previous Children 1st Referral Form) is live! It's available at: dph.georgia.gov/children1st🤝 This updated form is simpler, shorter, and more inclusive, making it easier to connect families with the services they need. The changes were made based on feedback from families, medical providers, Department of Public Health staff, and community partners.🗒 The form has been renamed from “Children 1st Referral Form” to “Child Health Referral Form” to more clearly reflect that this single form is to be used for all Child Health programs, including:• Babies Can’t Wait (BCW)• Children 1st• Children’s Medical Services (CMS)• Early Hearing Detection & Intervention (EHDI)• Maternal, Infant, & Early Childhood Home Visiting (MIECHV, where available)#childhealth #BabiesCantWait #children1st #Childrensmedicalservices #earlyhearingdetectionandintervention #maternalinfantandearlychildhoodhomevisiting #district2publichealth #DPH ...
