This department provides birth certificates and death certificates for individuals born in all counties in Georgia. By law, only certain persons are entitled to a certificate.


Birth Certificates

Who can request a birth certificate?
  • The person named on the certificate
  • The parents listed on the certificate
  • The authorized legal guardian (certified court order required), grandparent, adult sibling, adult child, or spouse of the person named on the certificate. Proof of relationship is required.
Where can I obtain a birth certificate in person?
  • Visit us at the Hall County Health Department.
  • Present a government issued ID (i.e. Driver’s License, Passport, State of Georgia ID)
  • If required, provide proof of relationship
  • Non-refundable search fee: $25.00.
    This fee includes one certified certificate, if found. Additional certified certificates for the same person ordered at the same time are 5.00 each. We accept Cash, Discover, MasterCard and Visa (cardholder must be present with ID). No personal checks are accepted.

Can I obtain a birth certificate by mail?

Yes.

Mail the following items to the address listed below:

  • Completed and signed Birth Certificate Application. All fields must be filled completely.
  • Money Order made payable to Hall County Vital Records for applicable fees ($25.00 for the first copy, and $5.00 for each additional copy requested for the same person at the same time).
    This fee is a non-refundable search fee for all birth certificates.
  • Self-addressed stamped envelope must be included.

Can I obtain a birth certificate online?

Online requests are processed through VitalChek, Inc, an independent company we have partnered with to provide clients with online service. To access VitalChek, visit their website at www.vitalcheck.com.

The cost for this service includes $25.00 for the first copy, $5.00 for each additional copy requested for the same person on the same request, and an additional processing fee charged by VitalChek.

All major credit cards are accepted, including American Express®, Discover®, MasterCard® or Visa®


Death Certificates

Who can request a death certificate?
  • The parents listed on the certificate
  • The authorized legal guardian (certified court order required), grandparent, adult sibling, adult child, or spouse of the person named on the certificate. Proof of relationship is required
  • Legal representative of the family members (Proof required)

*** Death certificates are only issued to applicants having a direct and tangible interest, primary family members or legal representatives of the family. It shall be the responsibility of the applicant to show proof of right or claim.

Where can I obtain a death certificate in person?
  • Visit us at the Hall County Health Department.

Some corrections to birth and death certificates, legitimations, and paternity acknowledgments can be initiated through this office by appointment.
